CARE FROM THE GET-GO

HHA: Home Health Aides
Particularly important for someone who requires regular supervision, medical equipment, or complex medications
Healthcare-related tasks.
PCA: Personal Care Attendants
Ideal for semi-independent older adults who need moderate support.
Provide care within homes and help with daily activities/tasks such as bathing, shopping, housework, and more.
PA: Personal Aide
Every individual can be a PA and it does not require any form of certification.
Friends and family members are the most common ones.
Difference between HHA & PCA/PA
HHAs can perform skilled tasks (including some health care/nursing duties).
PCAs and PAs may only perform non-skilled functions like cooking, cleaning, and assisting with activities of daily living."

PEDIATRICS SPECIAL-NEEDS SERVICES

When taking care of a child approved for OPWDD services, Anchor will provide you with PADS-certified, individually matched aides and behavioral specialists to assist you in bringing your child to their fullest potential
  • Facility age-out and transition.
    Social Workers and MSCs to
    ensure continuity of care

NHTD NURSING HOME TRANSITION & DIVERSION

The NHTD program is a Medicaid-funded program for people experiencing a neurological decline. The program provides the care of a specialized facility but from the convenience and warmth of home. Through the NHTD program, seniors can receive constant care and supervision around the clock.
  • 24-hour supervision with alternating
    shifts by qualified aides

    visits with accompanying aides

CDPAP PERSONAL ASSISTANTS

CDPAP allows you to choose a caregiver of your choice, including family members or friends with no prior experience. Anchor will help you apply and navigate the program so your loved ones maximize their full potential.

CARE WITHOUT DISRUPTIONS

Ongoing quality care and aide stability is our top priority. In the unlikely event that your caregiver is out of commission, a substitute aide will arrive at your door without any service interruption.

BILLING DESIGNED FOR YOU

As a provider of both Medicaid and private pay, we work with long-term care carriers to set you up with the best available care and identify the optimal coverage and reimbursement for each service.

CLUSTER CARE

To ensure that every case is staffed regardless of approved hours - even one hour daily, ‘Cluster care’ pools together cases by facility or neighborhood to provide consistent and ongoing care with no minimum hours required.
